47 year male with DM
PRESENTING COMPLAINTS:
C/O Pain Abdomen since a year.
C/O Diminision of vision in both eyes since 6 months.
C/O Pain over the small joints of hand and feet since a month.
HOPI:
A 45 year male presented to the OPD with complaints of epigastric abdominal pain after having a meal, followed by diffuse pain abdomen. Pain Abdomen relieved in between the meals and after having PPI medications.
History of gradual painless diminished vision since 7 months, not associated with glare.
History of small joint pains( MCP; DIP Joint) since a month not associated with redness or swelling.
PAST ILLNESS:
In 2021, he had acute onset of pain abdomen for which he sought for consultation and diagnosed to be having Diabetes Mellitus, for which he was started on life style modification.
PERSONAL HISTORY:
Moderately built and nourished.
Sleep adequate.
Appetite normal.
Bowel and bladder regular.
Occasional Alcoholic.
FAMILY HISTORY:
Father - Type II Diabetes Mellitus.
DRUG HISTORY:
He was on TAB. PANTOPRAZOLE + DOMOERIDONE.
GENERAL EXAMINATION:
Patient was conscious and coherent.
Afebrile; Temp: 98.4°F.
PR: 84bpm; RR: 17cpm; BP: 110/70 mmHg; SpO2: 98%@ RA.
CVS: S1,S2+; R/S: BAE+, Clear; P/A: Soft, Non tender, BS+; CNS: NFND.
COURSE IN THE HOSPITAL:
A 45 year male clinically presented with above mentioned complaints. Necessary investigations were done.
